A fillable PDF is a digital form that can be completed on your computer and sent electronically.

Here are some forms to help you communicate with others through your cancer journey. You are not required to fill out any of these forms, nor do you have to answer every question if you do.

Directions:  To use our fillable PDF forms, use your keyboard to fill out the document. When you are satisfied, select “Download” if you would like to send your form electronically. After download, the document will exist on your device, and only on your device. Now you can email or text it to the desired recipients or upload it to your health portal. If you would like to hand your form or “snail mail” it to the desired recipients, click on the three dots at the top right and select “Print this file.”

Alternatively, you can print a blank document and fill it out by hand—but know that your information is just as private when you fill out the form online. Nobody can see it but you and the people you choose to share it with. Your information is not, never will be, and never could be part of our website because you are not uploading it.

Support Registry Form

Most people need at least some help while in treatment for cancer, and often people want to help but don’t know how.  Studies show that people greatly underestimate how willing others are to help them and thus refuse sincere offers of assistance, but they shouldn’t; allowing others to help us makes life easier, makes both parties happier, and makes our relationships stronger. Still, communicating what you need help with can be awkward.  But just as a couple registers for wedding gifts, you can register for the kinds of support you need.  Fill out this form and then send it to friends, family, neighbors, coworkers, and anyone else who says, “Let me know if there’s anything I can do to help.”

About Me Form

This form might be helpful when you visit a doctor, especially if it’s your first appointment with that doctor.  Many patients have something they wish they could tell their doctor, but they aren’t asked.  Or they want their doctor to know something, but they feel uncomfortable saying it out loud.  Or they intend to ask the doctor something, but then they forget.  Fill out the “About Me” form and then print it and take it with you to your appointment or ask about uploading it to your health portal in advance.
